CommonHealth ACTION is building the Port Towns community leaders’ capacity to improve health outcomes by addressing the social determinants of health. Collectively, the Port Towns include Bladensburg, Colmar Manor, Cottage City, and Edmonston located in Prince George’s County, Maryland. The Community Health Initiative (CHI) is a partnership between the Port Towns community, Kaiser Permanente, Consumer Health Foundation, and other institutions, to change community conditions that increase the risk of obesity, chronic disease, and health inequities in the Port Towns. The Port Towns CHI is also a member of the national PLACE MATTERS Initiative. Through a 7-10 year funding commitment from Kaiser Permanente of the Mid-Atlantic States, the CHI focus is Healthy Eating, Active Living with the goal to reduce chronic illness related to poor nutrition and inactivity through addressing lack of access to healthy foods and low levels of physical activity. The framework acknowledges the critical role of poverty, education, racism, and other social determinants that are the root causes of poor health.
